From a lawyer. The best way to deal with this is to consult with a probate lawyer and file for a guardianship which is superior to a power of attorney. The judge will simply decide what is in the best interest of the incapacitated person. You will have a chance to get records from the person who is now handling their affairs and see whether or not they have handled things properly. Social workers can assist with power of attorney. The only people who cannot be witnesses are heirs. To find a good lawyer contact the state bar association of your state for a referral.
